REGISTRATION FOR 2021-2022 SCHOOL YEAR

Thank you for your interest in Aldersgate Christian Preschool.

Registration Fee is a non-refundable, yearly fee of $150 for a child or for $250 for a family. After you complete your registration forms online, you will be billed for your registration fee.

STEP ONE-

ALL PARENTS: Please read through the registration packet and complete the forms online from your family email. We will bill you your registration fee. Once you are paid your child will be placed into a class and you will recieve an email confirmation.

STEP TWO-

ALL PARENTS: Please fill out completely an immunization record for your child. If you already have a copy or can print it from your WA immunization record or medical account, please scan it in and email us or mail it.

In case you need it, a blank form is attached here. Please fill out and then email to: director@acpreschool.com.

These must be received by the first day of school for your child to start.

STEP THREE (IF APPLICABLE)-

For PARENTS with a child(ren) with ALLERGIES/ INTOLERANCES:

Please work with your pediatrician to fill out completely the allergy/intolerance report attached below if you do not have a current one. This must accompany any EpiPen, AUVI-Q or other medicine (like antihistamine) being sent in with your child.

Any student needing medicine cannot start school until we have both the medicine AND the allergy/intolerance report with the physician's signature.
